Vesical stone formation caused by a forgotten and migrated intrauterine contraceptive device: a case report with literature review

BackgroundAs widely known, an intrauterine device (IUD) is a small, typically T-shaped contraceptive device that offers long-term reversible birth control. However, neglecting to remove the IUD can lead to severe complications. This case report showcases an exceptional instance that underscores the potential risks associated with retention of an IUD.Case presentationThis case report documents a 40-year-old female who failed to remove her intrauterine device for eight years. Upon her visit to our clinic, she presented with increased urinary frequency, burning micturition, dysuria, and discomfort while emptying her bladder. Medical review consisting of ultrasound (KUB) and X-ray pelvis revealed the intrauterine device had migrated and was encrusted with a stone. We used Holmium laser lithotripsy to break down the stone, before using cystoscope to manually remove the intrauterine device. She made a rapid recovery without any complications.ConclusionsIn order to raise awareness about forgotten or misplaced IUDs and their complications, we are presenting a case of complete intravesical IUD migration at our clinic. This serves as an educational opportunity to shed light on this uncommon occurrence and emphasize the importance of timely removal and follow-up in patients using intrauterine devices (IUDs).

Migration of Intra-Uterine Devices.

Intrauterine devices (IUDs) are a widely used contraceptive. Possible complications from IUDs include failed insertion, pain, vasovagal reaction, infection, abnormal bleeding, and expulsion. Uterine perforation and migration of the IUD are rare complications occurring in approximately 1-2 per 1000 insertions. We executed a systematic review by reviewing all case reports and case series on IUD migration, published between December 2002 and December 2022. Our review indicates that about half of these patients present with pain and that a third are completely asymptomatic. The most common sites of migration are the intestine, bladder, and omentum. We found that the preferred method for removing the migrated IUD is laparoscopy. Generally, there are no lasting injuries after the removal of the migrated IUD, but occasionally, severe complications have been reported. Healthcare providers should be vigilant about this rare complication, especially in cases of painful insertion or the presence of other risk factors for perforation. When uterine perforation is diagnosed, it is advisable to remove the IUD to prevent severe complications.

Intravesical Migration of An Intrauterine Device: A Case Report

Intravesical migration of an intrauterine device (IUD) is an uncommon complication. We report here a case of IUD migration into the bladder in a 41-year-old patient who had been wearing an IUD for 3 years and who initially consulted us because of urinary burning and pollakiuria associated with episodes of haematuria. The diagnosis of intra-vesical migration was made by ultrasound and confirmed by cystoscopy. Endoscopic extraction of the IUD was performed successfully. Post-operative management was straightforward.

Two Natural Births with Unremoved Iuds Causing Bladder Stone Formation Due to IUD Migration: A Rare Case

Background: Intrauterine Device (IUD) Multiload 375 is a method of contraception widely used worldwide. One of the rare complications is that the IUD punctures the uterus and moves into the bladder. Common manifestations are recurrent urinary tract infections and bladder stones. A long-term and neglected IUD increases the risk of uterine perforation and migration to other organs. Diagnosis: Clinical history, Cystoscopy, Transvaginal Ultrasound (US) or Computed Tomography helps confirm the diagnosis. Case Report: We report successful surgical treatment of a 39-year-old woman who neglected to insert a Multiload 375 IUD for 16 years, but still experienced 2 subsequent vaginal deliveries and eventually developed UTI. Bladder stone formation caused by the passage of the Multiload 375 intrauterine device through the bladder. Conclusions: Urologists should consider bladder foreign bodies as a displaced IUD in women with recurrent lower urinary tract infections or bladder stones, with a history of IUD use. The moving IUD needs to be removed through cystoscopy, laparoscopy, or open surgery. Proper monitoring and education for women’s health before and after IUD insertion helps to detect complications in a timely manner.

Introduction and importanceImplantation of an intrauterine device (IUD) is a common method of contraception in Saudi Arabia. Although rare, IUD migration and colon perforation have been reported. The current report presented three cases of IUD migration into the colon and recto-uterine pouch. Methods and outcomesThe study included a series of three cases of migrated IUDs. The first case was a 25-year-old female, Gravida 2, Para 2 + 0, at 28 week-gestation, who presented with abdominal pain with a history of IUD placement that had not been removed or imaged before. The patient submitted to the caesarian section (CS), where IUD was found in the sigmoid colon. Elective laparoscopic removal of IUD with resection and primary repair of sigmoid colon was done later. The second case was a 37-year-old female, Gravida 1, Para 1 + 0, non-pregnant hypothyroidism, and a history of IUD placement. The patient got pregnant and gave birth through CS. She was then presented with abdominal pain and requested the removal of the IUD. On colonoscopy, IUD was seen in the pouch of Douglas with no evidence of a fistulous tract. IUD was removed through laparoscopy. The third case was a 47-year-old female, Gravida 14, Para 14 + 0, with a history of previous CS presented with a missing IUD that had been inserted 20 years ago after she had five pregnancies and subsequent deliveries. On colonoscopy, IUD was embedded on the wall of the transverse colon, and through abdominal surgery, IUD was removed by cutting through the colon and primary repair was done. Clinical discussionthe presentation of IUD migration cases was foundto vary according to the site of migration and type of IUD.however the cases are usually present with abdominal pain. An abdominal pelvic imaging with CT in these patients are essential in diagnosis.Retrival of migrating IUDs may be done through colonoscopy, laparoscopy, and in some cases with adhesion laparotomy is the solution. ConclusionAbdominal and pelvic CT scan are very important in the diagnosis and the localization of IUD. Elective colonoscopy and laparoscopy are successful management approaches for these cases.

Colouterine Fistula Caused by Intrauterine Device Migration : A Case Report

Intrauterine device (IUD) is simple and long-term medical device contraception. The IUD is a one of safety long duration contraception with several systemic side effects. But it can also cause morbidity by migration into another organ. IUD migration is a one of rare complication. We present a 28-year-old woman came to surgery department presented with hematoschezia when menstrual period since one month ago. Physical examiation showed hemodynamically stable and no abnormality. According to pelvic x-ray examination showed corpus allienum of IUD projected in pelvic cavum as high as foramen III-IV sacralis anterior. Patient also examined for CT-Scan that showed corpus allienum 1000-2000 HU density (T-shape IUD) in uteri cavum penetrate right posterolateral wall of uteri into sigmoid colon as high as S3 vertebrae followed by pneumoperitoneum of Douglas cavum. Colonoscopy showed rectal bleeding due to corpus allienum of expulsion of IUD. IUD extraction may lead to difficulties using colonoscopy so she planned to laparoscopic procedure. Possible laparoscopic options for IUDs embedded in the bowel include device extraction and intracorporeal suturing, or resection of the affected segment with primary anastomosis. Misplaced of Intrauterine Devices: The Endoscopic Management

Intra uterine device migration is a relatively rare event. The migration of the IUD in the surrounded viscera can be managed by endoscopy approach. Objectives: We reported our experience to determine the epidemiologic characteristics of patients that presented migrated IUD, to report clinical aspects and describe the laparoscopic management. Method: The study includes 10 patients reported to obstetrics and Gynecology I department of UHC Hassan II FES in the last 4 years (2018-2021). In all the patients a gynecological examination, ultrasound evaluation abdomen and pelvis were done to locate the misplaced IUD. If the device was found to be intrauterine, hysteroscopy was done to locate and retrieve device. In those patients in whom the IUD was confirmed to be extrauterine diagnostic laparoscopy was done. Results: The average of age was 34.5 years. Majority of patients were primipara (53.3%) and 46.6% were multipara. As far as timing of insertion is concerned 6 (60%) were postpartum, 4 (40%) were postmenstrual. The average of time interval between insertion and removal is 3, 12 years. Of the 10 misplaced intrauterine devices 4 were found partially embedded in uterine wall and 6 of the misplaced 1UD’s were found deeply embedded in the uterine cavity. After their location they were removed hysteroscopically One patient reported with 8 weeks pregnancy and misplaced IUD. The device was confirmed to be intrauterine on transvaginal sonography. She had full term normal delivery and IUD was removed after 6 weeks hysteroscopically. None of our patient had an extra uterine migration that required a laparoscopy.

Intravesical Stone Following IUD Trans Utero-Vesical

Background: Intravesical lithogenesis following IUD intrauterine device migration is a serious complication which has been rarely reported. Case Report: Here in, we report the case of 46 aged woman, 7th gestation and 7th parity, who started with periodic complains of pollakiuria, burning and urgenturia 6 months after IUD insertion. Ultrasonography and Plain abdominal X ray evoked an intra bladder T device witch branch was enclosed by lithiasis. Referred to the hospital for proper management, the diagnostic confirmation has been done by uretrocystoscopy. A cystolithotomy was performed and post operatory follow up has been uneventful. Rapid specialized management is advisable face to IUD cord missing, especially when this is associated to LUTS (low urinary tract syndrome).

Intra-vesical migration of IUD and the formation of stones are a rare complication. It can cause symptoms of the lower urinary tract. Imaging is crucial for diagnosis. Endourological management is the preferred option because the success rate is high with minimal morbidity. We present the management of two cases of intrauterine device migration forming an intra-vesical calculus with dysuria, suprapubic pain and hematuria.

Calculus Formation in Bladder from Migrated Intrauterine Devices

Migration of the intrauterine device (IUD) into the bladder has been a rare case. There were reported 31 cases of IUD migration into the bladder until 2006. Although IUD migration is asymptomatic, it should be removed to prevent complications such as pelvic abscess, bladder or intestinal rupture, and adhesion. A 52-year-old woman came to the urology clinic with pyuria since the previous 3 months. She had a history of IUD insertion in 1982; and two months later, she got pregnant. Since 2015, she has suffered from dysuria but has never been treated and has worsened in the past 3 months. On physical examination, tenderness was found in the suprapubic region. The results of urinalysis showed pyuria and hematuria. The ultrasonography findings, there were large bladder stones. An abdominal x-ray revealed the presence of a bladder stone with the IUD tail. Vesicolithotomy was performed and the IUD was found attached to an 11x7 cm bladder stone. The patient had a good postoperative condition without any special complications. The IUD in the bladder is a medium for forming secondary bladder stones. Most cases of IUD migration are caused by a lack of evaluation after the installation procedure. This case suggested that the physician should be more careful in carrying out the installation procedure. It is necessary to evaluate the location of the IUD after installation to prevent further patient complications.

Introduction: Ileal perforation due to intrauterine device (IUD) migration is rare situation, but severe complication that can occur years after the insertion. Depending on the location of the injured intestine. Case presentation: A 64-year-old female presented to the outpatient clinic with a dorsal pain the patient reported a history of a neglected spine trauma a x ray was performed showing a chance spine fracture of the D12 and a small metallic T shape foreign body in the right iliac fossa a CT scan showed an IUD like foreign body in the lumen of the terminal ileum. After these CT findings, the patient informed us that she had IUD inserted almost 34 years ago. The patient refused the surgery removal option. Conclusions: Perforation of IUD can be asymptomatic, although sometimes it can cause short-term or long-term symptoms. Penetrated IUDs should be removed whenever identified. For intra-abdominal penetrations, the laparoscopic or mini-laparotomy approach is a safe and appropriate method.

Acute Peritonitis Complicating Sigmoid Colon Perforation by Migrating Intrauterine Device: A Case Report and Review of The Literature

Introduction: Intrauterine device (IUD) migration is a rare situation which can cause some serious complication such as colon perforation and high risk of infection. Its treated surgically by laparoscopy or laparotomy depending of the difficulties and complications. Presentation of Case: We show the case of a 45 year old woman, G2 P2 carrying an IUD for 15 years, who consulted for abdominal pain and fever. Abdominal imaging revealed the presence of intraperitoneal effusion of medium abundance and pneumoperitoneum. Diagnostic laparoscopy was done and the IUD was found to be embedded in the wall of the sigmoid colon which was removed after laparotomy conversion by excision of the involved segment followed by the suturing and drainage. Conclusion: In rare cases IUD can migrate, still asymptomatic for years or cause sometimes serious complications like intestinal perforation or abscess and peritonitis, this risk must be avoided by treating all migrating IUD even asymptomatic ones.

Abstract Intra uterine devices (IUDs) is one of the most effective form of contraception available today. Uterine perforation and IUD migration is a rare but serious complication and is reported to occur in 0.05 to 13 cases out of 1000 IUCD placements. Migration of IUD to pelvis and lower abdominal organs following uterine perforation has been reported widely in literature. However far migration into the upper abdomen in very rare. Here we report a case asymptomatic peri-pancreatic migration of IUD. A 65-year-old lady being investigated for pneumonia was picked up to have an asymptomatic IUD lying in front of the pancreas. The patient recalled having a hormonal Mirena coil placed 12 years previously for menopausal symptoms. Routine 3-year general practitioner follow-up failed to detect the IUD on vaginal examination. The patient denied spontaneous IUD discharge. However, three subsequent pelvic ultrasound scans did not detect any retained IUD and she was discharged as unrecognized IUD expulsion. She remained asymptomatic for the next 12 years with no abdominal symptoms though she did report to the General Practitioner, multiple episodes of new onset hot flushes when opening bowels - which was managed conservatively with reassurance. It was decided to surgically remove the IUD. Intraoperatively, the IUD was found densely adherent to the lesser omentum close to the pancreas and stomach and was removed safely laparoscopically. The patient made a good postoperative overnight recovery and was discharged. Intra-abdominal migration of IUDs is a very rare complication of IUD insertion and when detected should be promptly treated with Laparoscopic removal.

Objective Intrauterine devices (IUDs) are globally one of the most popular methods of contraception. Uterine perforation is one of the most significant complications of IUD use and commonly occurs at the time of IUD insertion rather than presenting as delayed migration. This paper reports a series of 13 cases of displaced IUDs requiring retrieval by laparoscopy or laparotomy. All the IUDs were copper bearing and most perforations occurred immediately after IUD insertion. Cases In two patients with sigmoid colon injury and IUD penetration of the appendix, laparoscopic management had failed and laparotomy was necessary owing to severe obliteration of the pelvic cavity. In one patient laparotomy was the preferred surgical approach owing to acute bowel perforation. In the remaining patients, the displaced devices were successfully removed by laparoscopy. Conclusion Uterine perforation and IUD migration to the organs in the abdominopelvic cavity are serious complications of IUD insertion and can be successfully managed by laparoscopy, or by laparotomy in the presence of severe pelvic adhesions or unexpected complications.

The intrauterine device (IUD) is an effective, reversible, widely used contraception method. After RIA placement, rare, serious complications such as migration to abdominopelvic viscera, pelvis, bladder, rectum and colon can be observed. A 33-year-old female had a previous history of IUD insertion in an external center presented emergency service with abdominal pain. Due to the presence of acute abdominal findings, explorative laparotomy was performed. The exploration revealed a RIA with a limited abscess formation migrated to the sigmoid colon wall and mesocolon. IUD was excised with limited mesocolon of sigmoid and primary suturing was performed. The patient was discharged uneventfully on the 7th postoperative day. Transmigration of IUD to sigmoid colon is a rare condition. Physicians should keep in mind that patients with a history of IUD who had changes in bowel habits and chronic pelvic pain symptoms may be a sign of colonic migration of IUD.
